01 Risk first

Safety essentials

The information most likely to change a prescribing or dispensing decision.

Contraindications

  • [Domperidone] Hypersensitivity to PPIs.
  • Caution with rilpivirine and some antivirals (gastric pH). [Esomeprazole] Hypersensitivity to PPIs.
  • Caution with rilpivirine and some antivirals (gastric pH).

Precautions

  • Use lowest effective dose/shortest duration.
  • Long-term: hypomagnesaemia, B12 deficiency, increased GI infection risk, fracture signal, fundic polyps.
  • Masking of malignancy — investigate alarm features.
02 Point of care

Dosing matrix

Population and organ-function guidance, shown together for faster comparison.

Adult

Dosing is indication-, age-, weight- and organ-function-specific for this INN. Do not use class averages for high-risk patients. Confirm the exact regimen in the current SmPC and Kenya Standard Treatment Guidelines. Typical professional workflow: (1) confirm indication, (2) check renal/hepatic function, (3) screen interactions/allergies, (4) select dose/route/duration, (5) define monitoring.

Paediatric

Paediatric dosing is weight- and age-based; use a paediatric formulary / SmPC. Do not extrapolate adult tablets without calculation.

Renal

Rare AIN; otherwise minimal renal dosing issues.

  • CrCl 0–120: Confirm renal dosing in product SmPC / primary label.
Hepatic

Severe hepatic impairment: dose reduction for some agents.

03 Clinical use

Use, effects & interactions

Indications

  • Therapeutic use is agent- and indication-specific within the class (Proton pump inhibitor (PPI)).
  • Use according to culture results, national guidelines (Kenya STG/EML where applicable) and the current product SmPC.
  • Gastroesophageal reflux disease (GERD).

Adverse effects

  • [From component Domperidone] Raised prolactin concentrations hence likelihood of having gynaecomastia and galactorrhoea
  • reduced libido
  • hypersensitivity reactions
  • hyper- tensive crisis with phaeochromocytoma.
  • Fixed-dose/multi-ingredient product.
  • Clinical details partially inherited from component monographs: Domperidone, Esomeprazole.
  • Confirm combination SmPC for exact dosing.

Read complete mechanism

Combination product. Component mechanism (Domperidone): It is a dopamine antagonist, which stimulates gastric emptying, strengthens oesophageal sphincter contractions and enhances small intestinal transit. As an anti-emetic, it acts at chemoreceptor trigger zone hence not effective in vestibular disorders e.g. motion sickness.
